Criteria for Applicant Review

Applicants must have been diagnosed with breast cancer and can be in some phase of treatment and or recovery. Applicants must be a resident of the city of Dallas, Texas.

Additionally, an applicant will qualify for consideration if they meet the
Income/Asset Requirements AND/OR the Duration Requirements set forth below:

I. Income & Assets
Applicant must meet at least two of the following criteria:

  1. Total household income from all sources (including but not limited to wages, retirement pension, alimony, worker's compensation, Social Security, employer disability insurance, etc.) does not meet the sum of all financial obligations for the patient's determined treatment period.

  2. Household income during the illness has dropped or will drop more than 25% after the first month of diagnosis.

  3. Total household income is too high to qualify for government aid but still does not meet the sum of all financial obligations for the patient's determined treatment period.

  4. Individual is receiving federal or state government aid (including but not limited to, (SSI or SSDI), but cannot meet monthly financial obligations.

  5. Total cash assets equal less than the sum of all financial obligations for the determined treatment period.

II. Duration of Illness/Recurrence
In the alternative, applicants may also qualify for consideration by the Board to receive assistance if they meet the following criteria:

  1. The Applicant is currently undergoing treatment for a recurrence of her breast cancer.
  2. Due to the length and/or severity of the treatment, the Applicant will be unable to work or contribute to the household income for a period of at least four (4) months.

SMALL PEARLS OF HOPE will conduct a review of each individual's financial
situation as well as the duration of the individual’s illness to determine the greatest need. To assist the Panel in it’s decision, each applicant is asked to disclose and prioritize exactly what support they are asking for.

SMALL PEARLS OF HOPE will interview the applicant, and reserves the right to
interview the applicant's physician and the applicant's social worker (if applicable), to understand the full scope of need. SMALL PEARLS OF HOPE will then determine what funding, if any, will be available to assist the applicant in meeting her financial needs during this period.
